Advanced Physical Therapy

We offer early and late appointments to accommodate patients that may need those times due to work or family schedules.

New Patient Information

  • Please arrive 15-20 minutes early to register with the front office prior to your appointment time. Bring your driver's license, all insurance information and a referral from your physician (if you were given one). Your physician's office may fax a prescription directly to the appropriate office: 501-224-5460 (Little Rock); 501-758-5941 (North Little Rock); 501-776-9778 (Benton) or 501-941-3340 (Cabot).

Print and fill out the appropriate patient forms below and bring them with you to your first appointment. If you have any questions about which forms to complete, please contact our office at 501-224-5454.

Bring a list of all medications you are currently taking and any test results which may be applicable (MRI reports, nerve conduction reports, X-rays, etc.).

Your visit will last around an hour. Orthopedic patients should wear comfortable shoes and clothing. All evaluations are performed by a licensed physical therapist in a comfortable and private setting.

Please keep in mind that your appointment time is not an estimate. It is very important that you arrive on time for your appointment and be respectful of other patients' appointment times. If you are late for your appointment then your treatment may have to be shortened and you may not receive the full benefit of you treatment for that day. Please call ahead to let us know if you think you may be late for an appointment.

Most orthopedic patients are seen three times a week for 4-6 weeks and pelvic floor patients are typically seen twice weekly for 4-6 weeks. Every treatment is important and you will progress faster if you commit to attend all your appointments. If you must miss a scheduled appointment please call us as soon as you know this and we will do our best to reschedule your appointment.

Health Insurance

While we are not required to do so, as a courtesy to our patients, Advanced Physical Therapy will verify insurance benefits and file all insurances, both in-network and out-of-network. If your plan is out-of-network, we will help you weigh your options regarding physical therapy treatment. Please contact our front office staff to discuss any questions you may have regarding the specifics of your insurance policy.

All billing is performed in-house by our billing specialist, Melissa Ruby. Melissa can be reached at 501-224-1225 should you have questions regarding a bill or your account.
